Referring a patient to Amy Slabaugh, RDN

The California Business Code for dietitians requires me to have a referral in place from a physician before I can begin working a patient. You can fax my referral form to my office (see the form above) or you can use your own office’s referral protocol.

If you use your own referral process please be sure that each referral includes:

Provider’s name and NPI

Patient name and phone number

Diagnosis and ICD-10 code

Indication that this is a referral for nutrition counseling or medical nutrition therapy

What Patients Can Expect

Rates

  • Initial Assessment: 55 minutes – $225
  • Follow Up Session: Up to 45 minutes – $160

Insurance Coverage

Exact coverage for nutritional counseling varies from policy to policy.

I encourage patients to access my insurance benefits verification form to help them know what questions they should ask their insurance company to determine coverage and potential out of pocket costs.

In-Network:

I am in-network with United Healthcare and Aetna.

Out-Of-Network:

For patients with other health insurance you can pay for my services out of pocket and then receive a “superbill” to submit to your insurance company for potential reimbursement.

How Many Sessions?

Lasting health changes don’t happen overnight. While some patients choose ongoing support, most new patients see meaningful progress with 4–6 sessions.
This timeframe allows us to build realistic habits, adjust as needed, and create changes that actually stick—without extremes or quick fixes.

(Many patients notice improvements even sooner, but consistency is key for long-term results.)

Once a patient decides to move forward, I’ll create their Healthie account and guide them through the next steps so getting started feels simple and stress-free.

I love helping people rediscover the joy of eating while also helping them to feel good in their bodies. In nutrition counseling sessions with me you have my expert guidance in getting you from restricted diets and not feeling well to eating more freely and feeling good.

When you work with me we always start with a thorough assessment so I can see the whole picture of where you’ve been and what you’ve tried. From there we work collaboratively to make an action plan of steps you can take to get you feeling better and eating freely.
